Collaborative Data Collection Form – This form allows SEPAC members to provide details related to concerns, questions, accolades and issues in an easy to use format that will be shared with district administration leaders during quarterly meetings between the SEPAC Leadership Team and Pupil Services Department. The form also allows us to collect and save the shared information in an attempt to recognize and address any district wide patterns. Your name and other identifying information is optional.
